Changing Your Name on a Valid US Passport

To perform Name Change quickly, applicant must meet the following qualifications:

-- Applicant has US passport in his or her possession, AND;
-- Applicant has legally changed their name after most recent passport was issued AND;
-- Applicant's passport was issued within the past 15 years AND;
-- Applicant was 16 years or older when passport was issued.

Your Passport
|
|
|
Send Sierra PVS your expired Passport. We will return your old Passport along with your new passport. The new Passport will have a different Passport number.
If you do not have your Passport, you will need to apply for Replacing a Lost or Stolen Passport

Original Name Change Documents
|
|
|
Send us the original or certified copy of the marriage certificate (not a license) or a court decree ordering a name change or an adoption. The document must show the official seal of the issuing authority. All official documents will be returned to you with your amended passport.

If passport is GREATER than 1 year old: Click on the link at the bottom of this section (Section D) to create a DS-82 Application for Passport by Mail.

Print all pages of the DS-82 form, including the instructions, so that each page is printed only on one side of the paper. Do not print on both sides of any sheet of paper.
You must sign the completed DS-82 on the last page of the DS-82 form.

If Passport is LESS than 1 year old: Complete the DS-5504 US Passport Re- Application form.

Government Fees

A check or money order made payable to "U.S. Department of State" for $170. This fee may also be paid online via credit card when filling out our online order form below. If passport is LESS THAN 1 YEAR OLD AND YOU ARE COMPLETING THE DS-5504 FORM, THE CHECK OR MONEY ORDER AMOUNT SHOULD ONLY BE $60.
NOTE: If paying by check:
Please include your date of birth on the memo line of the check. Starter checks for new bank accounts are NOT acceptable.
Although it is recommended that your check have your name pre-printed on it, if submitting a business check or someone else's check, make sure to include your name, address and date of birth on the check. Money orders must contain your information on them.

Letter of Authorization
|

A Letter of Authorization gives us the authority to submit your application and pick-up your passport when issued. We are only allowed to submit a certain number of passports each day, and the additional names on the additional authorization letters are for our partner companies under whose name we may submit your application in order to provide the fastest available service.
NOTE: PLEASE SUBMIT 2 COPIES. Each copy MUST contain your original signature preferably in Blue Ink. Fax or photo copies of your signature are NOT acceptable - both letters must contain original signatures.

Proof of Departure
|
|
|
Submit a copy of your computer-generated itinerary or your ticket from an airline, travel agency, or online booking agency showing your departure from the U.S. FAX or photocopies are acceptable.
If you are unable to provide a trip itinerary, we can still assist you without one. It may take one or two additional days to process your request.

Passport Photos
|
|
|
You will be required to submit two (2) identical passport photographs. Photographs must be taken by a passport photographer. Homemade photos are not acceptable and most often cause a delay in passport issuance.

You must complete the Order Form. Please print two copies of the confirmation page. Include one copy in the envelope you ship to us and keep one copy for your records.
Processing times are business days and do not include shipping time. Keep this in mind when placing your order. Orders are filled based on departure date. For example, a traveler leaving in 10 calendar days that selects 5 - 7 day service may experience fulfillment times of up to 11 calendar days including shipping.
